Current COD students, faculty, and staff who already have a COD IDcan use this form to have their ID linked to their Library account. Please include the number at the bottom of the ID (it will start with 2130). Once activated, it can then be used as a Library card. Please allow 24 hours for processing. All fields are required.
If you do not yet have a COD ID and are registered for at least one in-person class, you can obtain an ID through Student Life (SSC 1101) or from the upper-level Library Circulation desk. If you are an Online-Only student, please use the form below to request an online-only ID.
